Your loved one must have Medicaid and have the EDCD Waiver/CCC + to receive personal care services in the home. We would need a copy of an UAI (Uniform Assessment Instrument) Form and a discharge DMAS (Department of Medical Assistance Services) 225 Form faxed to 757-292-4293 in Hampton Roads or 757-302-0024 in Eastern Shore.

If you or your loved one needs help with bathing, dressing, grooming, toileting, transferring, ambulation, feeding, laundry, light meal prep, medication reminders and/or accompany doctor’s appointments then your loved one must be.

—- (Adults) screened by Social Services (in your city) and the Health Dept Registered Nurse. You must call and ask for an initial visit for home care-based services.

—-(Children up to age 18) you must contact the Health Dept Registered Nurse (in your city) and request a visit for home care-based services.

Yes, but you must be a PCA (personal care aide), CNA (certified nurse assistant), or a NA (nurse aide).

Love Thy Neighbor will provide FREE PCA training so you can receive compensation for what you do everyday anyway. Please call the office for more information and the Registered Nurse’s availability for training classes.

Medicaid will allow a spouse or a parent of a minor child to be compensated as an aide. You would be considered a Legally Responsible Individual (LRI) which means you will only receive up to 40 hours weekly for personal care services and you will not be allowed to use respite hours. The client’s respite hours will terminate. Please review Medicaid’s website regarding LRI’s.

A Registered Nurse schedules a call to conduct an initial free visit and they complete their assessment to determine how many hours they feel an aide needs to be in the home with the potential client. With the help of the client/family, the RN completes her assessment and submits all required documentation to the client’s MCO (managed care organization) and then a decision is made regarding the personal care hours / respite care hours.

Once we receive an authorization from the client’s insurance company, we contact the client or POA to inform them. If the client is not happy with the hours approved, they have the right to appeal. Contact the office for further information.
